Service Coordination

Service Coordination addresses one of the most common frustrations in social services: people with complex needs being bounced from agency to agency, retelling their story each time and falling through the cracks in between. EPIC promotes integrated service delivery so that housing, behavioral health, victim advocacy, and other supports work in concert rather than in parallel.

In practice, this means designated coordinators who serve as a single point of contact for clients with overlapping needs, shared case planning across partner agencies (with the client’s consent), and warm handoffs that ensure no one is left to navigate the system alone. It also means working at a systems level, convening providers, mapping gaps, and reducing duplication, so improvements benefit not just one client but everyone who walks through the door.
